Financial Liability

Your pet is important to us. Because we care about your pet’s safety and well-being, we wat to assure you that every effort will be made to make your pet’s visit as pleasant as possible. Due to the various conditions of some puppies (i.e. matting, temperament, nervousness, aggression, medical conditions, etc.), and in the best interest of your pet, we request permission to obtain immediate veterinary treatment should it become necessary. I am financially responsible for any harm to any person(s) or pet(s) that is determined to have been caused by my pet(s); such determination will be made by the management of PoochTrim after a careful review of the circumstances. The Owner’s liability and responsibility will extend to cover any and all medical and/or Veterinary costs resulting from their pet’s behaviour.  PoochTrim will use a licensed veterinary centre for emergency veterinary care. I hereby grant PoochTrim permission to seek emergency veterinary care from the above veterinary centre. I further agree that I am financially responsible for any medical treatment my pet(s) receive(s) while and/or resulting from attending PoochTrim.

I hereby release and agree to save and hold harmless, the PoochTrim, it’s directors, shareholders, employees, assistance’s, members from any liability, claims, suits, actions, loss, injury, or damage which I or my pet(s) may sustain or which may be caused in any way by my pet(s). I specifically, without limitation, agree to fully indemnify PoochTrim for any and all such liability, claims, suits, actions, losses, injury or damage.

Matted Coat Waiver

A PoochTrim staff member has looked at your dog and has determined that their coat is tangled or matted. The welfare and safety of your dog is of the utmost importance to PoochTrim. I understand that a mat is a mass of entangled hair, which must be brushed, combed, or otherwise removed from a dog’s coat. The more severe mats can actually draw the dog’s skin up into the clippers while being groomed. Due to the nature of matted fur, the more severe the matting, the greater the risk of injury. In these cases, it is not humanely possible to brush to comb the mats out and the dog must be shaved down. A matting fee will be charged. I further understand and agree that in admitting my dog to PoochTrim, there is a remote possibility that even though reasonable care and precautions are followed, by the staff of PoochTrim that and injury could occur.

If a medical emergency should arise while your dog is attending our facility, it is critical that we are able to provide medical treatment quickly. PoochTrim has made arrangements for emergency medical care with the following veterinary clinic: McKenzie Veterinary Services 250-727-2125.

In the event of a medical emergency arising, as a result of the matted condition, I agree to allow PoochTrim to seek medical care from the above veterinary centre. I further agree that I am financially responsible for any medical treatment my dog receives as a result of a medical emergency while attending PoochTrim. I certify that I have read and understand this waiver. I agree to abide by and accept all the terms, conditions, and statements of this waiver.

Senior Pet Medical Form

Your pet is important to us. Because we care, we want to assure you that every effort will be made to make your senior pet’s visit as pleasant as possible. Occasionally, the grooming process will expose hidden medical conditions or aggravate current ones. This can occur during or after grooming. The welfare and safety of your dog is of the utmost importance to us. In the best interest of your pet, we request your permission to obtain immediate veterinary treatment for your pet, should it become necessary. I hereby grant PoochTrim permission to seek emergency veterinary care for my pet. I also realize that there is a greater risk of injury to senior pets during the grooming process. With senior pets the risk of heart attacks, during the grooming process, increase with age. PoochTrim has made arrangements for emergency medical care with the following veterinary clinic:  McKenzie Veterinary Services 250-727-2125 I acknowledge these risks and I will not hold PoochTrim responsible for any accidental injury to or death of my pet during the grooming procedure.
